Release Forms
The following should be used when bringing a guest (Non-Member) or minor on a Fremont HOG sponsored ride. Please complete the required forms, and submit them to the lead Road Captain for that scheduled event.
Adult Event Release Form - This should be used by anyone over the age of 18, who is not a member of Fremont HOG, that will be riding as your guest.
Minor Event Release Form - This should be completed by the parent/guardian of the minor child who will riding as your guest.
Minor Risk Acknowledgement Form - This form is also required for any guest under the age of 12 that will be riding as your guest.
